Remote Patient Monitoring Solutions

Remote Patient Monitoring Services

Keeping Physicians & Patients Connected

The telehealth industry is rapidly evolving, and clear segments are being defined that align to specific requirements in the continuum of care. Remote Patient Monitoring for patients with chronic diseases is one area that shows the most promise, with a projected $1 billion market. In 2019 CMS created new reimbursement opportunities for connected care services that enable providers to manage and coordinate patient care remotely.
Remote Patient Monitoring (RPM) can be used to deliver healthcare services to patients regardless of their physical location or condition. Centro Healthcare’s Remote Patient Monitoring Solutions combines cutting-edge technology, reliable clinical staff, and the increased affordability of embedded communication devices to advance seamless, continuous provider care while easing the care burden for patients.

Remote Patient Monitoring Reimbursements

The recent expansion and unbundling of reimbursable services by CMS present several opportunities for providers to submit claims and receive reimbursement from Medicare. Remote patient monitoring has received new Medicare reimbursement codes, all effective January 1st, 2019

CPT 99454 – RPM Device Supply

CPT 99457 – RPM Periodic Monitoring

HCPCS G2012 – Virtual Check-Ins

$ 15 Monthly Payment

In addition, the American Medical Association (AMA) has recently announced six new codes supporting RPM for 2020. These are 99421, 99422, 99423, 98970, 98971 and 98972. Along the way you can count on Centro Healthcare to support your RPM practice to maximize new revenues while providing value to your patients.

Benefits of Outsourcing Remote Patient Monitoring (RPM)

Our Remote Patient Monitoring services transform traditional telehealth and remote care monitoring models to deliver a solution with comprehensive benefits to all parties involved.

INCREASE REVENUE STREAMS

Earn new recurring revenue and create new profit center through Medicare’s RPM codes 99453, 99454, and 99457 which present a valuable opportunity for ACOs to improve patient care and increase practice revenue by providing beneficial patient monitoring services.

SUPERLATIVE SIMPLICITY

With far less complexity than traditional dedicated telehealth devices, payers and providers can utilize remote care on a much greater scale with Centro Healthcare’s RPM services

IMPROVE PATIENT EXPERIENCE

RPM provides patients with higher levels of education and support by giving them information and feedback about their personal conditions daily, educating them and providing support. In addition to increasing patient assurance, as constant monitoring can give patients a peace of mind that any potential issues will be identified in a timely manner.

MEET CMS MANDATES

Stay in compliance with CMS’ requirements and guidelines through Centro Healthcare’s comprehensive auditable reports

IMPROVE HEALTH OUTCOMES

Telehealth-enabled clinical programs contribute to reduced hospital admissions and readmissions, reduced length of hospital stay, fewer emergency department visits, reduced mortality and lower costs, and help prevent and/or limit illness severity, thereby contributing to improved health outcomes. And since RPM allows patients to complete basic health testing on their own, healthcare professionals are allowed to treat more patients.

MEET MACRA’S QUALITY MEASURES

Year 2 of MACRA’s QPP includes changes to the Clinical Practice Improvement Activities performance category and the Advancing Care Information performance category that incentivize the use of RPM services. So, practitioners who use RPM to collect real-time PGHD and allow patients to access and/or transmit their data in or to the CEHRT, or incorporate PGHD into their CEHRT, can increase their performance scores in both the ACI and the IA categories.

INCREASE PATIENT ENGAGEMENT

RPM allows patients to play a crucial role in managing and understanding their own health conditions as well as provides more frequent touchpoints among clinicians, caregivers, patients and their families.
